Job Description


Performs a variety of functions and technical procedures necessary for patient admission, assessment, diagnosis, treatment, follow-up, and outcome evaluation. Participates in performance improvement, research, and educational activities.

Job Responsibility

 

  • Takes initial patient histories and performs physical examinations.
  • Initiates, documents, and communicates the plan of care and follows up to ensure complete screening and preparation of patients.
  • Performs daily patient rounds.
  • Writes orders for medications, laboratory work, and diagnostic tests.
  • Interprets laboratory and test results.
  • Confers with attending physicians, residents, nursing staff, and/or other care providers to ensure optimum quality of patient care.
  • Upon discharge, notes discharge summaries, prescriptions, and any referrals on patient charts.
  • Informs patients of the necessary post-discharge care instructions.
  • Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.

 

Job Qualification

 

  • National Board Certification as Nurse Practitioner or Physicians Assistant required.
  • Current license to practice as a Nurse Practitioner or Physician’s Assistant in New York State.
  • Current BLS certification, required.

Position Overview:

Under the direction of, and working with, the Chief Medical Officer, The Associate Medical Director of this long-term care skilled nursing facility is responsible for planning, review, supervision and control of the medical and health services of the facility. The Associate Medical Director assists in the formulation of policies, methods and procedures, and prescribes standards for treatment, diagnosis, control and prevention of illness, as well as, supervising methods of delivery and evaluating patient care.

 

Responsibilities:

  • Assists in the organization and coordination of physician services as they relate to patient care quality, and reduction of rehospitalization.
  • Serves as liaison between Department of Medical Affairs/Chief Medical Officer and Medical Staff, oversees on-call coverage schedule.
  • Participates in administrative decision making and recommends and approves relevant policies and procedures and will be responsible for communicating issues and concerns to system clinical leadership when warranted.
  • With the Chief Medical Officer, establishes and implements policies, procedures, and guidelines designed to assure the provision of adequate, comprehensive services and will also monitor, evaluate, and improve performance of medical services through performance improvement program.
  • Demonstrates proficiency in the knowledge of regulatory compliance based on, however not limited to the New York State Department of Health, Center of Medicare and Medicaid Services that relate to patient care and related services.
  • Works with medical staff and administration to ensure high value and high quality of care is delivered to patients with a focus on improving the patient experience, improving the health of the population, and maintaining fiscal responsibility and will be a part of the multi-departmental approach in improving the case mix index.
  • Serves as Primary Care Physician and provides direct patient care to a panel of sixty (60) residents.
  • Provides coverage for Chief Medical Officer and Medical leadership for the facility on the occasion of CMO’s absence.


 

Requirements:

Graduation from an approved school of medicine and completion of an approved residency in a specialty and Board Certified and licensed to practice in New York State; and experience in the field of specialty in a responsible medical and clinical administrative capacity.

Departmental Preference: LTC Medical Leadership Experience, Geriatric Board Certified/Eligible, AMDA Certified Medical Director, Palliative Care Certification, Post-Acute EMR

Job Description for TJH - Employed Geriatrician at Jamaica Hospital Medical Center

 

  • Inpatient consultation and follow-up of patients with geriatrics problems
  • Participation in Geriatrics trauma and orthopedics programs
  • Participation in the Ambulatory Care Geriatrics clinic, 1 half day clinic per week
  • Participation in the TJH Axel office (faculty practice), 1 half-day per day, with increase in time as practice grows
  • Patient panel at the long term care facility
  • Administrative tasks as needed by Director of Geriatrics for Geriatrics division
  • Resident and medical student teaching during above clinical activities
  • Lectures and participation in case conferences on a regular basis as part of the resident and medical student education program
