Job Summary

The Mount Sinai Health System is seeking a physician to provide Post-Acute Care and Long-Term Care services at The New Jewish Home Skilled Nursing Facility in New York City.

The Division of Hospital Medicine (DHM) is recruiting a physician to care for post-acute and long-term care patients at The New Jewish Home on Manhattan’s Upper West Side. This is a unique opportunity to join the Mount Sinai Department of Medicine, Hospitalist Division, while being based at one of New York’s leading skilled nursing facilities.

The New Jewish Home is among the largest and most innovative providers of facility-based and community-based long-term care services in New York, offering both post-acute rehabilitation and long-term care. The position provides opportunities to collaborate with hospitalists throughout the Mount Sinai Health System on scholarly projects, work with Geriatric Medicine fellows and medical students, and participate in medical education, quality improvement, and research initiatives.

This role offers an excellent work-life balance with a Monday through Friday daytime schedule from 9:00 AM to 5:00 PM.

Qualifications

• Board eligible or board certified in Internal Medicine or Family Medicine
• Certification in Geriatrics and/or Hospice and Palliative Medicine is preferred, but not required

Reports To

Facility Medical Director

Key Responsibilities

• Complete comprehensive admission evaluations for new patients
• Manage chronic medical conditions to optimize functional and medical recovery
• Identify and address new medical conditions in a timely manner
• Collaborate effectively with patients, families, and interdisciplinary care teams
• Participate in patient and family meetings to support communication and discharge planning
• Address barriers to care and adjust goals as needed
• Ensure smooth care transitions through direct physician-to-physician communication
• Complete and document advance care planning, including MOLST and related documentation
• Comply with all applicable federal, state, and local regulations, as well as facility policies and procedures

Attending Staff Physician for SNF in ROCHESTER

Advance Practice Provider – Jewish Home Physician House Calls - Full Time GENERAL SCOPE: The Physician House Calls (PHC) Physician Assistant / Advanced Practice Provider (APP; Nurse Practitioner (NP) or Physician Assistant (PA)) provides comprehensive primary medical care for the Physician House Calls (PHC) program to home bound older adults in their own residence under the guidance of the Vice President of Medical Services and Chief Medical Officer in collaboration with other members of the medical and interdisciplinary team. This position requires a high degree of independence, personal responsibility, and collaboration with other members of the interdisciplinary team, patients, their caregivers/family, and other service providers. This program aims to bring person-centered care to frail older adults in their place of residence and minimize the risk of requiring emergency services and hospitalization. EDUCATION / QUALIFICATION: Nurse Practitioner (Master or Doctorate degree or equivalent) in Geriatrics, Family or Adult primary care. Physician Assistant (Master’s degree or equivalent) with relevant experience and current, unrestricted license to practice in New York State. Current, unrestricted New York State Driver License, automobile and automobile insurance required as well as current and appropriate required certification.

Attending Staff Physician – Jewish Home of Rochester - Full Time. The Attending Staff Physician is the attending of record for assigned residents. The Attending Physician provides direction and coordination of medical care for assigned long term care (LTC) and short-term rehabilitation program (SRP) residents of the Jewish Home. The Attending Physician is a valued member of the interdisciplinary care team reporting directly to the Vice President of Medical Services / Chief Medical Officer. The attending physician works collaboratively with residents, families, staff, and external providers and services to ensure excellent person-centered medical care. EDUCATION / QUALIFICATION: Doctor of allopathic or osteopathic medicine. Licensed to practice medicine in New York State. Board Certified in family medicine or internal medicine. Added qualifications and/or board certification in geriatrics and/or hospice and palliative medicine are highly desirable. Certified Medical Director (CMD) Certification by the Post-Acute and Long-Term Care Medical Association is desirable.
